Change your name with DVLA (Driving Licence & V5C) | Trusted Deed Poll
If you’ve changed your name by deed poll, you’ll usually want to update your driving licence and your V5C log book(vehicle registration certificate). This guide explains what to do and what to send.
Step-by-step: change the name on your driving licence

Get the right form (D1 or D2).

Complete the form with your new details.

Send DVLA:

your completed form
-
your driving licence
-
original proof of name change (your deed poll)

Post it to DVLA:

Car/motorbike: DVLA, Swansea, SA99 1BN

Lorry/bus: DVLA, Swansea, SA99 1BR

You can still drive while you wait, and it’s free to change your name.
Important: DVLA says don’t send photocopies or laminated certificates.
Step-by-step: change the name on your V5C log book
DVLA says you must also update your V5C if you change your name.

Write your new name in the correct section:

New-style V5C: Section 3 (write full name, not initials)

Old-style V5C: Section 6 (write full name, not initials — and don’t tick “new keeper”)

Include:

a letter explaining why the name needs changing
-
proof of name change (your deed poll) if you’re legally changing your name

Send the whole V5C to the DVLA address shown on the V5C for that section.
If your V5C is lost: DVLA says you’ll need a V62 plus a letter and a £25 fee.
